- Affidavit on non-judicial Stamp Paper of requisite value by all the claimant(s) duly affirmed before Metropolitan/1st Class Magistrate or a Notary Public or any authority duly authorized to affirm or to take affidavit on oath.
- All are subject to the condition that the employee is enrolled with the firm.
